Métis Nation of Ontario: Helping Frontline Workers Help Their Clients
The Métis Nation of Ontario (MNO) represents the interests of Métis people and communities across the province. The Healing and Wellness branch of MNO delivers community-based programs and services to Métis communities, and helps its members navigate the health system. To support this work, MNO needed a reliable, up-to-date directory of health services across Ontario.
Thehealthline.ca was the perfect solution – comprehensive, accurate, up-to-date, and easy to search. To ensure frontline workers made the best possible use of the resource, staffers at thehealthline.ca Information Network (THL) worked with MNO staff to develop and deliver an online training program. In all, 60 MNO frontline workers were trained. At the same time, listings for the MNO and its regional offices were improved and updated on www.thehealthline.ca.

Behavioural Supports Ontario: Making Data Collection Easier
Funded by the Ministry of Health, Behavioural Supports Ontario (BSO) supports frontline workers in managing older adults with responsive behaviours such as aggression, wandering, and agitation. There are BSO programs in each of the 14 Local Health Integration Networks across Ontario, supporting workers in long-term care homes, hospitals, retirement homes, adult day programs, emergency response services, and other settings.
Rigorous evaluation is a critical part of the BSO program. In the South West that means collecting data from 78 long-term care homes and many other sites. Health Chat, a tool created by thehealthline.ca Information Network, was the ideal place to submit data electronically. “Health Chat creates a lot more efficiency,” says Faizal Dasu, Program Analyst with South West BSO. “We’re able to review the data as it comes in and as a result we can better understand what’s happening across the region.” Having a common electronic platform also helps more consistent data, says Kelly Kummerfield, BSO Program Lead.
BSO also uses Health Chat to provide resources and encourage discussion among providers. “It’s a great place to share success stories,” Kummerfield says. “I don’t think we’d be as nimble, communicative, efficient and connected without Health Chat. It has really enabled us.”
South West Assess and Restore

South West Assess and Restore Project: Connecting to the Right Services
“Assess and Restore” is a provincial initiative designed to support older adults who may be at risk of ending up hospital or losing their independence. A simple, screening tool has been developed that helps healthcare providers determine the level of risk for each individual, so that they can be connected to the most appropriate services and supports.
The South West Assess and Restore team needed a tool that would enable providers to link quickly and easily to local services for each risk level score. They turned to thehealthline.ca Information Network. “Thehealthline.ca was already very visible in this LHIN,” explains Kelly McIntyre Muddle, South West Assess and Restore project lead. “Because they’ve been involved in so many different projects in the healthcare system, they really understood what we were doing and were able to take our ideas and run with them. And the database is comprehensive, so we were confident that we wouldn’t be missing anything.”
Thehealthline.ca staffers are now helping the Assess and Restore team test the new website with pilot Assess and Restore sites, including EMS, primary care, emergency departments and physiotherapy clinics. “We’re getting a lot of positive feedback,” says McIntyre Muddle. “People like the idea of the site making it easy to find services.” The next step, she says, is a pathway to information for patients and caregivers.
